A unique opportunity to be part of a new grant funded Community Health Worker (CHW) program working with individuals and families experiencing challenges related to maternal child health or chronic disease management/prevention. Responsibilities involve client case management including assessment, planning, advocacy, follow-up, and home visitation. CHWs will be stationed in identified partner clinic settings throughout the community and will receive client referrals from providers who believe case management will improve health outcomes. The CHWs will assist clients to follow through with provider recommendations and appointments, develop individualized health plans, connect with needed community resources, and help to access health insurance. Referred clients in maternal and child health will include pregnant and postpartum individuals and families who are uninsured, underinsured, or experiencing barriers to care. Referred clients in chronic disease management will include those experiencing health issues related to high blood pressure, cardiovascular disease, diabetes, obesity, and other chronic conditions. CHWs will work closely with the program supervisor and multidisciplinary clinical staff to ensure all client needs are adequately met, appropriate health care and community referrals are made, and individualized health plans are progressing.

Minimum Qualifications
Graduation from high school or equivalent and six months of experience with local, state, and federal assistance programs including health care and social service systems; or any equivalent combination of training and experience that provides the desirable knowledge, abilities, and skills.

Necessary Special Requirements
Proficiency in translating and speaking the language of the minority population to which assigned, if required by the position to which assigned.
Certification as a Breastfeeding educator prior to completion of the probationary period, if required by the position to which assigned.
Possession of a valid driver’s license when operating a vehicle is necessary to the satisfactory performance of assigned duties.

Evening and limited weekend hours are possible; majority of hours from 8 a.m. to 4:30 p.m., Monday – Friday.
